Support Coordination

The Leisure Company’s experienced Support Co-Ordinators will work with you, to assist you in strengthening your ability to connect to, and coordinate, your supports.

This involves working together with you and your family/carer to understand your plan, identify what you expect from services, and how you would like your support delivered.  

Support Coordination includes, but is not limited to:  

• Understanding your NDIS Plan.  
• Connecting with Supports and Services;
• Establish and identify your Supports.  
• Reporting to the NDIA and assisting with meetings, supporting participants and families/carers with the NDIS reviews, following up reports   

You must have Support Coordination in your NDIS plan to access this service.
